John D. Cochrane is the editor of the journal E-Healthcare Connections and editor emeritus and founder of the Integrated Healthcare Report. He was also the founder and architect of the popular Integrated Healthcare Symposium series. He has also been active in providing strategic consulting services to physicians and hospitals. Prior to the founding of symposium, publication and consulting firms in 1992, Mr Cochrane was the executive vice president for UniMed America, the physician network division of UniHealth America and Sr. V.P. of Operations & Development for UniHealthAmerica Ventures, which included home care, pre- and post-hospital technologies, mobile diagnostic operations, MedGroup Management (IPA), the CareAmerica HMO, sugar-free diabetic centers and ElderMed America. Prior to that he directed the Management Services & Education Division of the California Hospital Association and held executive operations positions in hospitals and medical groups. In addition to chairing many large scale symposiums, Mr Cochrane is a frequent speaker at hospital & medical group leadership functions and board retreats.

Pacificare had Tillinghast-Towers Perrin test a unique home monitoring and chronic disease management system over a six month period to learn whether it improved the efficiency and quality of care for CHF patients. The AlereNet System, with its DayLink Monitor, allows the electronic exchange of patient data 24/7.Cardiac nurses access the data via a web browser and analyze trends. The study showed improved quality, high member satisfaction and reduced hospitalization and ER use Ð at the same time it demonstrated a high level of physician satisfaction.

Using Tele-Web Technology to Address the Critical Success Factors of Population Disease Management

Accordant's Tele-Web platform integrates a content-rich Internet site with a call center staffed by teams of registered nurses and social workers. Accordant's focus is on patients with complex chronic diseases such as multiple sclerosis, cystic fibrosis, and systemic lupus. In late January, Accordant's web site was recognized by the Disease Management Association as having the best Internet application in disease management. In addition to disease-specific content, the site offers AccordantLive, an opportunity for patients to interact with national experts. The side also offers "communities," nurse-led chats, and message boards. There are also personal assessment tools and 24/7 support.

LifeMasters Supported SelfCare, Inc. just announced the results of a year-long study designed to test the effectiveness of the personal computer as a healthcare delivery tool to access the company's Web-enabled disease management programs. The study, which included three groups of patients with congestive heart failure (CHF), was successful in demonstrating reduced hospitalizations and shorter hospital stays. Additional findings show improved compliance, lifestyle modification, and cost savings. The study also found that with training and coaching, patients (average age 79) were able to use the personal computer and the Internet as a healthcare tool, as well as to increase socialization.

Dr. Molly Joel Coye is the founder of the Health Technology Center, a non-profit organization sponsored by the Institute for the Future. She was previously the Director of the West Coast office for The Lewin Group, and also directed product development and marketing for HealthDesk. She was Executive Vice President for Managed Care at the Good Samaritan Health System in Santa Clara. From 1991 to 1993, she was the Director of the California Dept. of Health Services. Dr. Coye was also Commissioner of Health for New Jersey from 1986 to 1990. Dr. Coye is a member of the Institute of Medicine and Quality of Health Care in America Committee.
