PRECONFERENCE SESSION I: UNDERSTANDING AND PREPARING FOR COMMUNITY HEALTH NEEDS ASSESSMENT (Required for Population Health Training Program)

Specific provisions of the Affordable Care Act encourage hospital organizations, governmental public health agencies, and other stakeholders to accelerate community health improvement by conducting triennial community health needs assessments (CHNA) and adopting related implementation strategies that address priority health needs.

This interactive session will provide an overview of the issues and challenges in coordinating and implementing CHNAs, including a presentation outlining the ACA requirements; an overview of best practices; developing meaningful outcome measures and related data collection; successful models and case studies; and a moderated discussion panel.

This session will be led by a panel of experts, including industry leaders and faculty from Thomas Jefferson University.

Drew Harris, DPM, MPH
Program Director, Masters of Health Policy, Jefferson School of Population Health, Philadelphia, PA (Moderator)

    Speaker Bio

    Dr. Drew Harris is an Assistant Professor and Director of the Health Policy and Population Health programs at the Thomas Jefferson University School of Population Health (JSPH) in Philadelphia, PA. His academic interest is the complex interplay between public health, medical care and public policy. At JSPH, he lead the team developing the MS in Population Health program and teaches the Population Health and Health Policy core courses.

    Dr. Harris is a health policy researcher exploring the impact of vaccine mandate policy on New Jersey vaccination rates. He founded the NJ Public Health Institute and served on the Executive Board of the American Public Health Association. He is an active public health advocate who testifies regularly on health policy issues. Through his op-eds and essays in the Philadelphia Inquirer Field Clinic blog, Wall Street Journal's Experts Panel and other publications, he informs the public by telling the stories behind the health research.

    Speaker Bio

    Dr. Brawer PhD, MPH is the Associate Director of the Thomas Jefferson University and Hospital's Center for Urban Health and an Assistant professor in the Department of Family and Community Medicine in Jefferson's Sidney Kimmel Medical College. As Co-Director of SKMC's College Within the College Population Health Program, she has helped integrate public health content and experiential opportunities into the core curriculum. She also teaches in the MPH program at Jefferson's School of Population Health.

    Dr. Brawer has practiced public health in the Philadelphia area for more than two decades developing strong relationships with area public health related coalitions, agencies and organizations. Dr. Brawer is responsible for developing and conducting the Community Health Needs Assessment for Thomas Jefferson University Hospitals.

    Nadim is president and founding Board member of the Rapid Results Institute and senior partner at Schaffer Consulting.

    The Institute partners with government agencies and non-profit organizations to solve major societal problems. Current partnerships include working with the Department of Veterans Affairs to end veteran homelessness, and working with Nesta to accelerate the delivery of integrated health and social care in England.

    Nadim's work has been featured in the New York Times. He was named by Foreign Policy Magazine among the top 100 Global Thinkers. And he served as a Donaldson Fellow at the Yale School of Management.

Marti Macchi, MEd, MPH
Senior Consultant for Diabetes, National Association of Chronic Disease Directors, Gilbert, AZ

    Speaker Bio

    As the Senior Consultant for Diabetes with the National Association of Chronic Disease Directors (NACDD) Ms. Macchi oversees all diabetes-related projects and initiatives. Prior to joining NACDD, Ms. Macchi served as the Director of Special Studies for the Kansas Department of Health and Environment where she was responsible for state initiatives involving policy, systems and environmental change approaches aimed at diabetes prevention and control, heart disease and stroke prevention, arthritis, physical activity, nutrition and obesity prevention. Additionally, Ms. Macchi's career experience has focused on disease prevention, health promotion and community wellness initiatives in both the clinical and community settings. She has supervised a large staff of clinicians focusing on obesity treatment, cardiac and stroke rehabilitation, diabetes management, physical and occupational therapy and various other health promotion programs.

    The National Association of Chronic Disease Directors (NACDD) is a non-profit Public Health organization for chronic disease program directors of each state and U.S. territory. Founded in 1988, NACDD connects more than 4,000 members to advocate for preventive policies and programs, encourage knowledge sharing and develop partnerships for health promotion. Since its founding, NACDD has been a national leader in mobilizing efforts to reduce chronic diseases and their associated risk factors through state and community-based prevention strategies.

PRECONFERENCE SESSION II: OUTCOMES MEASUREMENT FOR DUMMIES...AND SMARTIES

Whether for medical homes, disease management or employer-based clinics, outcomes report-writing and report-reading are skills which few have mastered. This interactive and educational workshop will make you an expert in interpreting numbers and promises in proposals and outcomes. You'll learn how to prepare your own reports, and to distinguish valid results from invalid claims made by many vendors and consultants.

By the end of this session, participants will:

  • Be able to spot "face invalidity" in outcomes reports in all major sectors of population health
  • Recognize biases that cause results to be overstated or studies to be invalid
  • Understand how to estimate outcomes/savings from population health, using simple but valid and transparent techniques

The self-test may be taken at this time or at your convenience (either at or after the conference). Responses may be handed in or emailed. This test may also be taken for Care Innovations Validation Institute Certification. An additional $250 fee applies and the test must be passed.

Note: If you wish to take the test and also attend Preconference III, register for both and you may take the test at your convenience, rather than at 10:30.

The first half of this session will be a joint program with Preconference Session II. The learning objectives are the same (see session description above).

The second half of this session, from 10:15 - noon, will cover examples and issues specific to medical homes. Examples will be drawn from the private sector, commercial health plans, Vermont and, of course, North Carolina.

    Speaker Bio

    David B. Nash, MD, MBA is the Founding Dean of the Jefferson School of Population Health where he is the Dr. Raymond C. and Doris N. Grandon Professor of Health Policy. He is a board certified internist and is internationally recognized for his work in outcomes management, medical staff development and quality-of-care. He has authored more than 100 articles in major journals, edited 22 books, and is editor-in-chief of four major national journals.
Marci Nielsen, PhD, MPH
Chief Executive Officer, Patient-Centered Primary Care Collaborative, Washington, DC (Co Chair)

    Speaker Bio

    Dr. Marci Nielsen is Chief Executive Officer of the Patient-Centered Primary Care Collaborative (PCPCC). She previously served as Vice Chancellor for Public Affairs and Associate Professor within the Department of Health Policy and Management at the University of Kansas School of Medicine, where she was twice awarded Professor of the Year by her students. She also served on the Board of Directors of the Health Care Foundation of Greater Kansas City, TransforMED, LLC (a wholly owned, non-profit subsidiary of the American Academy of Family Physicians), and the MidAmerica Coalition on Health Care. She also has been a committee member on the Institute of Medicine's Leading Health Indicators for Healthy People 2020 and Living Well with Chronic Illness: A Call for Public Health Action. She serves as the first public (non-physician) member on the American Board of Family Medicine. Prior to rejoining the KU faculty, she was appointed by then-Governor Kathleen Sebelius to oversee Kansas' health care agency, the Kansas Health Policy Authority (KHPA). At the national level, she has worked as a legislative assistant to U.S Senator Bob Kerrey (D-Nebraska) and as the health lobbyist and assistant director of legislation for the AFL-CIO. She earned her MPH degree from George Washington University and her PhD in Health Policy and Management from the Johns Hopkins School of Public Health.

Stephen K. Klasko, MD, MBA

    Speaker Bio

    On June 20, 2013, Stephen K. Klasko, MD, MBA, was appointed President and CEO, Thomas Jefferson University and Jefferson Health System. Dr. Klasko, who has served as Dean of the Morsani College of Medicine at the University of South Florida and CEO of USF Health since 2004, began his tenure at Jefferson in September 2013. Jefferson is the largest freestanding academic medical center in Philadelphia, with over 12,000 employees and 3,700 students.

    Dr. Klasko's optimism and enthusiasm for healthcare and information technology will be guiding forces at Jefferson to build on the institution's almost two-century-old reputation for integrated healthcare delivery, interdisciplinary education and investigation from the laboratory to the bedside.

    Christopher Kay is Senior Vice President and Chief Innovation Officer at Humana. In this role, he works closely with internal business leaders, as well as outside partners, to design, test, and operationalize game-changing innovations. He is a member of the Management Team, which sets the strategic direction for the company.

    Chris is a keen innovator with a passion for creating new businesses in large global organizations and for launching products and services that enhance the consumer experience. He most recently served as Managing Director and CEO of Citi Ventures, Citigroup's global corporate venturing arm. At Citi Ventures, he pioneered efforts to simplify and improve the banking experience for Citi's millions of customers through use of digital, mobile, data, and other customer-centric solutions. Chris and his team also invested in and worked closely with a portfolio of companies to accelerate time-to-market of new capabilities at Citi.

    Prior to joining Citi in 2007, Chris held several leadership positions at Target over a 12-year period, ranging from leader of large merchandising departments to store operations and property development.

The Synergy Between Population Health and the Medical Home

Roy A. Beveridge, MD
Chief Medical Officer, Humana, Inc., Louisville, KY

    Speaker Bio

    Dr. Roy Beveridge joined Humana in 2013 as Senior Vice President and Chief Medical Officer. He is responsible for developing and implementing Humana's clinical strategy, with an emphasis on advancing the company's integrated care delivery model.

    Dr. Beveridge is known for creating collaborative environments among physician communities and providing thought leadership around population health. He is a member of Humana's Management Team, which sets the strategic direction for the company.

    Previously, Dr. Beveridge served as Chief Medical Officer for McKesson Specialty Health, a Dallas-based subsidiary of McKesson Inc. Dr. Beveridge also was the Co-Director of the Bone Marrow Transplant Program at INOVA Fairfax Hospital in Falls Church, Virginia. He has published extensively in the fields of medical oncology, quality design and population health.
Rushika Fernandopulle, MD, MPP
Founder and Chief Executive Officer, Iora Health, Cambridge, MA

    Speaker Bio

    Dr. Rushika Fernandopulle is the Founder and Chief Executive Officer of Iora Health in Cambridge, MA, and one of the great innovators in primary care models. He was the first Executive Director of the Harvard Interfaculty Program for Health Systems Improvement, and served as a Managing Director of the Advisory Board Company. He serves on the faculty and earned his AB, MD, and MPP degrees from Harvard University and completed his clinical training at the Massachusetts General Hospital.
Thomas Graf, MD
Chief Medical Officer for Population Health and Longitudinal Care Service Lines, Geisinger Health System, Danville, PA

    Speaker Bio

    Dr. Thomas Graf is the CMO for Population Health and Longitudinal Care Service Lines for Geisinger Health System. Dr. Graf is responsible for the Value Re-Engineering of the Care Continuum and other population health initiatives for Geisinger including the ACO portfolio and with CMS, the Physician Group Practice Transitions Demonstration and Bundled Payments for Care Improvement. He leads the Community Practice, Internal Medicine, Pediatrics, Psychiatry and Care Continuum Service Lines in coordinating and accelerating population health related activities across 22 counties in central and northeast Pennsylvania. He is recognized nationally as a leader in medical home and post-acute care redesign.
Leslie Kelly Hall
Senior Vice President of Policy, Healthwise, Boise, ID

    Speaker Bio

    Leslie Kelly Hall is Senior Vice President of Healthwise, where she combines her experience as a former hospital administrator and vision as a consumer/patient advocate to fuel patient engagement in health care. As senior vice president she guides policy efforts for health information technology, standards, interoperability, privacy, and security that will help people engage in their health. Hall is widely recognized as a leader in health care information technology, and as a result of her appointment by former HHS Secretary Sebelius, she currently serves as a consumer engagement expert on several HHS Federal Advisory Committees.
Fred Goldstein
Interim Executive Director, Population Health Alliance; Founder, Accountable Health, LLC, Jacksonville, FL (Moderator)

    Speaker Bio

    Fred Goldstein is the Founder and President of Accountable Health, LLC and he serves as the Chair of the Board and Interim Executive Director of the Population Health Alliance. He has over 25 years of senior management experience in the health care industry encompassing Population Health, Disease Management and Wellness, HMO, hospital and physician group operations, strategic planning, mHealth, marketing, product development, network development, and government affairs. Fred serves on the Editorial Board of the Population Health Management journal.

    He received a BA in Zoology from U.C. Berkeley and an MS in Health Care Administration from Trinity University in San Antonio Texas.
